Patient Responsibilities

Understand that insurance coverage is a contract between you and your provider. While we will attempt to help you receive the maximum reimbursement possible, it’s your responsibility to fully understand what procedures are covered under your policy. Bring a copy of your available benefits that are listed on your insurance company’s website if you need clarification of what treatments are included.
Please have ready all necessary documentation during your initial dental appointment. This may include: employer, name of your insurance company, group number, Social Security number, and alternative employee ID if provided.
On the day of your treatment, we ask that you pay your deductible and the estimated patient portion based on the percentage of coverage provided in your specific dental plan.
Please respond quickly to any correspondence you may receive from your insurance provider. Answering and addressing any requests or concerns from your provider will help you receive the fullest reimbursement of benefits possible.
Once all insurance claims have been processed, please pay the remainder of your account balance with Horizon Dental. We happily accept all major credit cards for your convenience.
Please inform us of any change in insurance coverage or employer before your next appointment.

Our Responsibilities
  • We will quickly and accurately update your insurance coverage in our records based on the information provided by your insurance provider.
  • We will quickly and accurately file all insurance claims on your behalf.
  • We will provide you an accurate and up-to-date balance of your account once all insurance claims have been paid.
  • It can take between 30 to 45 days for an insurance claim to be fully processed for reimbursement.
